Texas licensed nurse practitioner to provide chronic risk assessments and care team follow-up for senior populations.

Working within a structured care team model, the ability to assess patients over the long-term and establish plans of care to include psycho-social, education, monitoring, and regularly planned interventions to routinely account for the health status of high risk and multiple complex chronic patients. Practice will combine both clinical and virtual visits and assessments.

Ability to work within a shared patient care team structure. Exceptional relational skills to build compliance and establish trust with patients. Experience in population health management environment preferred.
